The State of Illinois is conducting an audit to ensure only eligible dependents are covered on State health plans. If you have dependents covered on a State health plan, and do not respond to this audit, your dependent(s) could lose their health insurance coverage!
You must complete the two steps below. Once complete, you will receive a postcard from HMS Employer Solutions stating that no further action is necessary. Please contact HMS at 855-596-3354 if you have any questions.
There are two important steps to complete the eligibility audit:
- ALL EMPLOYEES with covered dependents must submit copies (not originals) of the required documents that validate each dependent’s eligibility. See the Required Documents list on the back of your letter from HMS.
- If the SSN box on your form is blank: In addition to the above, you must also provide a copy of your dependent’s Social Security card, or the letter from the Social Security Administration stating that your dependent is ineligible for an SSN. CMS is required by state and federal law to collect a Social Security Number for every covered individual on the benefit plan.
Carefully read the letter and FAQs that you received from HMS, as they provide instructions for submitting copies (not originals) of documents and contact information for HMS. You must provide documentation for your dependent(s) to HMS by October 30, 2015.
What if I never received a letter and FAQs?
If you did not receive the HMS mailing and you have dependents enrolled in a State health plan: Contact HMS at 855-596-3354, 7 a.m.to 7 p.m. CT, Monday through Friday, and request a copy of your letter.

This eligibility audit is being conducted by the Illinois Department of Central Management Services (CMS). CMS requires that all questions be directed to HMS, and not to CMS or the University.
