Eligible dependents can be reinstated if the Fall Certification deadline of October 31, 2010 was missed. If the certification form is provided within 30 days of the November 1, 2010 termination date, coverage will be reinstated retroactive to that date. If the certification form is not provided within that 30-day period, coverage can be reinstated within 60 days of a correlating qualifying event, or anytime, if the employee already insures 2 or more dependents.
