Previous notice was sent on December 19, 2011 to Coventry Health Care HMO (formerly PersonalCare HMO) members announcing the contract termination with Christie Clinic.
To review the complete information provided in the previous announcement please visit NESSIE. Members of Coventry Health Care HMO can choose a new PCP or select a new health plan until midnight on February 29, 2012.
If your Primary Care Physician (PCP) was a Christie Clinic doctor, you have two options:
- You may remain with Coventry Health Care HMO and choose a new PCP. If you choose this option, you may accept the physician that Coventry assigned to you in their letter or you can select another physician by calling Coventry at 800-431-1211 or accessing their website.
- You may change to another managed care health plan or the Quality Care Health Plan. This must be done before midnight on February 29, 2012 and can be done online in NESSIE.
With the qualifying event of “Change in Health Plan due to Primary Care Physician Leaving the Network,” the only change you may make is a change to your health plan.
Employees changing health plans after January 1, 2012 but prior to March 1, 2012 will have coverage with the new health plan effective the date of the request. Any health plan change due to this qualifying event must be completed before midnight on February 29, 2012 (for this change, a copy of the Coventry letter is not required).
If you made a health plan change in the past few days, it may not be reflected in the system yet. You may disregard this notice if you already made your change. If you wish you may contact your campus UPB Benefits office at the contact information provided below to verify that your change has been received.
