Re: Christie Clinic Termination with Coventry HMO Effective 12/31/2011
Coventry Health Care HMO (formerly PersonalCare HMO) sent its HMO members a letter earlier this month regarding the termination of the contract between Coventry Health Care and Christie Clinic effective December 31, 2011. If your Primary Care Physician (PCP) was a Christie Clinic doctor, you have the following options:
- You may remain with Coventry Health Care HMO. If you choose this option, you may accept the physician that Coventry assigned to you in their letter or you can select another physician by calling Coventry at 800-431-1211 or accessing their website.
- You may change to another managed care health plan. This must be done prior to March 1, 2012 and this can be done online in NESSIE. Other managed care health plans are: Health Alliance HMO, Health Alliance Illinois HMO, Coventry OAP, HealthLink OAP, HMO Illinois, and BlueAdvantage HMO. HealthLink OAP - Christie Clinic doctors and Provena Covenant Hospital are Tier 1 providers.

Health Alliance HMO - Christie Clinic physicians, Provena Covenant Hospital, Carle Clinic physicians, and Carle Foundation Hospital are in-network under this plan.
- You may change to the Quality Care Health Plan (QCHP). This must be done prior to March 1, 2012 and this can be done online in NESSIE. See the FY12 Benefit Choice Options booklet at for information about this plan including the monthly cost.
- Quality Care Health Plan (QCHP) - Christie Clinic and Provena Covenant Hospital are preferred providers.
If you choose to change health plans, a copy of the letter you received from Coventry Health Care must be provided to your UPB Benefits Services office as proof of your notification that a qualifying event has occurred. Any health plan change due to this qualifying event must be completed prior to March 1, 2012.
Employees who select a new health plan prior to or on January 1, 2012, will have coverage under the new health plan effective January 1st. Employees changing health plans after January 1, 2012 but prior to March 1, 2012 will have coverage effective the date of the request.
