If you have dependents covered under a State health plan and have not responded to or have not completed the eligibility audit, you must respond and provide all required documentation to validate the eligibility of your dependent(s) by November 19.
Failure to submit all required documentation will result in the State terminating health/vision, dental, and life insurance for your dependent(s) (i.e., spouse/partner, children) effective December 1st.
