The University of Illinois Foundation has moved the Gift Transmittal Forms to a web format, as of Saturday, May 21.  All gift transmittal forms work best in a current version of Chrome, Firefox, Microsoft Edge or Safari.  The gift transmittal forms are available through UIF Online, under Forms and in TED, via the UIF Help tab.
The information requested on the forms remains the same; however, the forms now have separate sites for Cash, Check and Gift in Kind, which can be bookmarked, just like other frequently used web sites. The Gift Transmittal Instructions are available via a link from each form.

The Securities and Wire gift transmittal forms are utilized by UIF only.  Questions regarding securities or wire gift types should be directed to the Director of Cash Receipts, at 217.244.0352.
