Urgent Notice: Benefit Choice NESSIE Electronic Enrollments End June 14
The Benefit Choice NESSIE electronic enrollment application option will close at 10 p.m. on Thursday, June 14 to facilitate a timely transfer of Benefit Choice transactions to carriers. Due to the extended Benefit Choice Period, carriers will have a short time frame to notify providers of new participants and print and mail plan I. D. and MyFBMC Visa cards to employees before July 1.
Employees are highly encouraged to use NESSIE Benefit Choice Enrollment on or before Thursday, June 14 so changes can be submitted and processed electronically.
After 10:00 p.m. on June 14, the NESSIE Benefit Choice enrollment page will contain links to paper Insurance, MCAP and DCAP enrollment forms to allow employees to continue to make Benefit Choice elections through 11:59 p.m. on June 15. Completed paper forms must be received by UPB Benefits or postmarked no later than June 15. They can be faxed to 217-244-3135 or dropped off at a UPB Benefits office by close of business on June 15.
Benefit Choice changes made on paper will be processed manually and employees most likely will not receive I. D. cards prior to July 1. All eligible health, dental, vision and prescription services received on or after July 1 will be covered once the CMS and carrier membership systems have been updated.
Employees have until June 25 to submit relationship or opt-out documentation. If documentation is not received within that 10-day period, transactions will be voided. Optional life insurance requests approved before July 1 will be effective on July 1. Requests approved after July 1, are effective either the 1st or 16th day of the month following the approval date.
Employees who enroll in MCAP, DCAP or make insurance coverage changes are asked to check the CMS verification form and July Earnings Statements to verify those changes.
