A Health Plan Special Enrollment Period is available from October 10-28, 2011; the only change that can be made at this time is a change from one health plan to another health plan. Changes become effective December 1, 2011.
If employees do not wish to make a health plan change NO ACTION IS REQUIRED.
Employees making a health plan change can access the online enrollment form in NESSIE, available from October 10 to midnight on October 28, 2011.
Important information regarding health plan changes:
** Change your health plan carrier and you will be required to meet any and all deductibles under the new health plan, as well as under the prescription drug plan, even if you already met the deductibles under your previous plans.
** If you change from a plan that uses the MEDCO prescription benefit manager to another plan that also uses MEDCO, you will still need to meet a new prescription deductible. Plans that use Medco are HealthLink OAP, Health Alliance Illinois, PersonalCare OAP, and the Quality Care Health Plan (QCHP).
** For employees who elect an HMO plan, a Primary Care Physician (PCP) must be selected and identification number entered into the NESSIE enrollment form. On page 2 of the Benefit Choice Booklet find links to the HMO provider directories.
** If you elect the Quality Care Health Plan (QCHP) or an OAP plan, a Primary Care Physician (PCP) is NOT required.
** Transition Care: Members or dependents involved in an on-going course of treatment or who have entered the third trimester of pregnancy should contact their new plan to coordinate the transition of services for treatment under their prior carrier.
Prior to December 1, 2011 employees who change their health plan will receive a CMS verification form confirming the change. Employees making changes will receive health care ID cards from their new carrier and also may be able to print temporary ID cards from the carrier's website. Please review your December Earnings Statement in NESSIE to verify that any changes to premium deductions have been updated.

The following changes cannot be made during this period but you may be eligible to make these changes if experiencing a Qualifying Event.
** Opt In or Out of Health Coverage
** Enroll or terminate Dental/Life Coverage
** Enroll or terminate Dependent Health/Dental Coverage
** Enroll or make changes to Dependent Care (DCAP) or Medical Care Accounts (MCAP)
