Also available are:
Health Alliance HMO
Health Alliance Illinois HMO

Quality Care Health Plan (QCHP)
To select one of the plans listed above you may do so by taking one of the following actions:
1) If you already selected a plan in NESSIE that is different from the plan that you are currently a member of see instructions below to reset your Benefit Choice selection.
2) If you have not made a selection and you want to stay in the plan that you are currently a member of you should make no choice (take no action) in NESSIE during the current Benefit Choice Period.
3) To choose one of the plans above or change from a selection you made previously, you may do so in NESSIE.
How can I reset my Benefit Choice selection if I already made a choice?
Resetting your form clears out all previously made Benefit Choice changes for the FY2012 plan year, effectively indicating "no choice made". This means it will reset any changes (including opt-out) to health, dental, dependents covered, and/or life insurance. The only plans unaffected by a "reset" are the flexible spending accounts (DCAP and MCAP).
If you want to reset your form:
Click on the Benefit Choice application at https://nessie.uihr.uillinois.edu/cf/bchoic/login.cfm and sign in. Then click on "Reset Benefit Choice Form"(click only once). Be sure that you are using this method only to reset your form. Then please select "View Last Submitted Benefit Choice Form" to verify your selection in all of your plans. Be sure to make any changes to dental, dependents covered and life insurance if necessary.
What if the plan I want has not yet signed a 90-day contract?
As of June 16, Humana Health Plan HMO and Humana-Winnebago HMO have not signed 90-day contracts with the State. You may wish to wait on making your selection, but you must make a choice of the available plans by June 17. Participants who previously chose or are current members of these plans also have the option to fill out a paper form and include a first and second plan choice on that form. First choice may be a non-contracted plan (i.e., Humana Health Plan HMO and Humana-Winnebago HMO), but the second or alternate choice must be a contracted plan (i.e., Health Alliance HMO, Health Alliance Illinois HMO, HealthLink OAP, HMO Illinois, BlueAdvantage HMO, QCHP, PersonalCare HMO and PersonalCare OAP). Participants in the non-contracted plans who make no choice by June 17 will default into the Quality Care Health Plan (QCHP).
The paper enrollment form is available at https://nessie.uihr.uillinois.edu/pdf/benefits/BenefitChoiceFY12EnrollmentForm.pdf. To submit the form, hand deliver to your campus Benefits Services office or fax to 217-244-3135.
Where do I find the PCP/NPI number?
Go to the CMS website at http://www.state.il.us/cms/3_servicese_ben_choice/state-ben.htm and click on your health plan link to find your PCP/NPI number.
What can I do in a Special Enrollment Period?
CMS stated that they are committed to holding a special enrollment period at some point after Benefit Choice concludes, which will allow employees a second chance to make their healthcare plan decision. However, per IRS rules, MCAP/DCAP changes will not be permitted during the special enrollment period.
