The University is correcting calendar year 2013 to exclude the value of same-sex health coverage for employees that have the required documentation on file. To be eligible to exclude the value of same-sex health coverage for calendar year 2013, employees married prior to September 16, 2013 must have a Group Insurance Enrollment form and marriage certificate on file with UPB Benefits Services as of November 14, 2013. This is the date required by the State of Illinois Department of Central Management Services (“CMS”). Employees married on or after September 16, 2013 have 60 days to submit an enrollment change.
Employees who wish to file a refund claim for income taxes should use Form 1040X , Amended U.S. Individual Income Tax Return.
Employees who wish to file a refund claim for gift or estate taxes should file Form 843 , Claim for Refund and Request for Abatement. Attach a statement to your Form 843 stating that your employer is not refunding your portion of the FICA tax.
