Before-you-begin instructions
Save yourself time. Follow these steps:
- Verify that the name printed on your current, government form of identity matches what is prepped for your UI Health (university system) records.
Why? You cannot receive your i-card if the name, date of birth, and legal sex on your government ID do not match what is in system records.
- Follow the photo requirements! Your photo is for small ID card, not an online profile. Lighting, position, and framing matter.
Why? Faulty photos will be rejected. Emailed photos are not accepted. After three photos are rejected, your only option is to to visit the UI Health ID Office to have your photo taken.
- Submit your photo early and check its status in three days.
Why? To allow time for photo review. The number of photos submitted increases significantly from May to September on all three U of I campuses. Your photo is in a queue, and once reviewed, it could be rejected.
