You have special permission to submit a photo online for potential use on a UI Health ID card, subject to rules and a deadline:
- Rules: Your photo must meet these linked photo terms and conditions. Photos can be rejected!
- Deadline: Your photo must be submitted and approved by 11:59 pm on Sunday, December 15.
Online photo submission is a valued, convenient option but it does not preclude you from obtaining your UI Health ID. See the "Card pickup instructions" further on this page.
Photo instructions
Follow these photo submission tips:
- Log in from a PC, not a phone. Choose Chrome or Firefox for your browser if possible.
- Don't use a "high resolution" pic from a phone.
- Make sure your photo has these qualities:
- The pic includes only from the top of your head to the base of your neck. No more is needed!
- The background is empty, very plain, and much lighter than the color of your hair.
- The photo is bright and clear, so that your eyes and features are easy to see.
- Monitor the status of your photo (approved or denied) by logging in again anytime before the deadline given above.
- Denied? You may submit another photo online if the deadline is not past.
- Approved? You cannot submit a different photo online once one has been approved.

- Emailed photos are not accepted.
ID pickup instructions
In order to claim your ID, you must present a government-issued, accepted proof of identity. Your pickup location will differ based on the date:
- Before December 21, visit UIC ID Center West. (Do not visit ID Center East!)
- After December 21, visit the UI Health ID Office. The ID Office will be open December 23, 27, and 30 and also on January 2 and 3.
