UAFR has recently updated the Banner Fund, Program, Index (FPI) Code Request Form to implement required updates for FY25 business. See below for the necessary steps you must take when requesting a new fund, program, or index code:
- Download the most recent version of the Banner Fund, Program, Index (FPI) Code Request Form from UAFR's Forms webpage.
- It's very important to download a new version of the FPI form for each request that you submit (as opposed to re-using an old version). We now use robotic software to perform the data entry related to these requests, and the software is not able to read previous versions of the form.
- To ensure you have the most recent version of the FPI form, refer to the Form Version date in the upper left-hand corner of the FPI Code Request tab. The most recent version of this form has a Form Version date of 07/01/2024.
- If you are using an FPI form with a Form Version date preceding this date, then you’ll need to download the updated FPI form prior to proceeding with your request.
- Complete the sections of the form which pertain to your request. Keep in mind that some sections of the form may not be applicable (and thus do not need to be completed), depending on the nature of your request.
- As a reminder, the FPI form now has conditional formatting on the FPI Code Request tab which will highlight the fields that we need you to complete. To ensure this conditional formatting works properly, you must select the appropriate answer from the drop-down menu of each field within question #4 of the General Information section.
- For example, if you are submitting a request for a new program code, then you’d need to answer Yes within the Program drop-down menu of question #4 in the General Information section. Once you do that, the required entry fields within each section of the FPI Code Request tab will be shaded in blue.
- Once your FPI form is complete, send it to the applicable email address as outlined in rows 9 – 10 of the FPI Code Request tab. The applicable staff will then review your request and will contact you if any questions arise.
