Do you ever need to request a new fund, program, or index code to properly account for your various financial activities? If so, see below for some helpful tips:
- Download the most recent version of the Banner Fund, Program, Index (FPI) Code Request Form from UAFR's Forms webpage.
- It's very important to download a new version of the form for each request that you submit (as opposed to re-using an old version of the form), as this form is frequently updated.
- To ensure you have the most recent version of this form, refer to the Form Version date in the upper left-hand corner of the FPI Code Request tab. The most recent version of this form has a date of 10/7/22. If you are using a form with a date preceding 10/7/22, then you’ll need to download the updated form.
- Complete the applicable sections of the form which pertain to your request. Keep in mind that some sections of the form may not be applicable to your request (and thus do not need to be completed).
- For example, if you are simply submitting a request for a new program code, then you’d only need to complete sections I (General Information), III (Request a New Program Code), and V (Submitted By) of the FPI Code Request tab. You would not need to complete sections II (Request a New Fund Code) or IV (Request a New Index Code).
- Once your form is complete, email it to uas@uillinois.edu. UAFR will review your request and will contact you if any questions arise.
Finally, if you have another type of request (such as a request to improve the title of an existing program code or a request to terminate a fund code), you can simply send an email directly to uas@uillinois.edu with details explaining your request. UAFR will then review your request and will let you know if there are any questions or concerns.
